On 25th May 2018, the General Protection Regulation (GDPR) came into effect. This replaced the previous Data Protection Act. The GDPR brings with it greater accountability for those who handle personal data in terms of its collection, storage and management.
To help comply with GDPR consent requirements, I need to confirm that you agree to me holding data on you so that I can communicate with you by email, post, telephone, Skype, Zoom, FaceTime or other mutually agreed video call applications and provide my services to you. You have the right to withdraw this consent and also to request to see information I hold about you at any time.
My work as a Sophrology Practitioner, Mindfulness Teacher and Coach has always involved a high level of privacy and confidentiality. I respect the confidentiality of personal information I may learn or hold about you or your health/circumstances.
I am committed to protecting your privacy.
I would never abuse your trust in giving me your data, except if the information is required by law or required to comply with judicial proceedings, court order or legal process. I am required to hold records for 7 years so as to comply with professional, legal and insurance requirements.
With your consent, I will only be collecting and recording information such as your name, address, telephone number, email address, date of birth, gender, marital status, general health information including lifestyle or work history if applicable.
